early termination fee reimbursement form not working

i signed up for 5G home internet on 2/17/24 up until today the early termination fee reimbursement form is not working. I cannot fill in my name , order number, email address and upload my pdf . The link goes to the form but the form is disable and i can't click on anything .
I have been calling customer service with zero progress except for them telling me : can you try again tomorrow the site might be down today "
- 2/23 - one hour on the phone after 25 minutes on hold : site is down, please try tomorrow
-2/24- 1.5 hours on the phone : i placed a ticket , i will call you tomorrow
-2/25 - 2 hours on the phone : nothing on a ticket , it has not been done , looks like it is a turn around time of 10 days for a ticket to be resolved . i will call you back when this is done .
-3/16- after over 3 weeks of no follow up call from verizon customer service , i called again with 25 minutes on hold and an hour on the phone with a rep who told me just keep trying . then told me there is nothing she can do about it
-3/17 - called verizon customer service again this time i'm on the phone for 1 hour and 50 minutes , was transferred to 5 reps . i was told " i placed a ticket, this is so weird another customer in Kansas is also not able to use the reimbursement form , this ticket will take about 2-3 days to be done . sorry for the experience you are having . "
to add on to this , i went to 2 different stores one in tacoma, wa and one in puyallup, wa asking if they could help with the reimbursement submission both stores told me to contact customer service . They were not able to access the form on their tablets at the store either .
anyone has similar issue and was able to resolve ? please help
