turning in a phone

My 19 year old daughter that has her own plan called Verizon and ask them if she was to switch carriers  what does she do with her phone and how much would she have to pay. They told her you will pay nothing if you ship the phone back to us. They said they would mail her the box and shipping label. So she switched carriers. Well the box and shipping label never came and when she would call to ask why they were rude and wanted my pin for my account that has nothing to do with her account or her phone and she told them no. So I called and asked why they would they need my pin for my account when it had nothing to do with her account and the lady said either give her your pin or go to the store. I ask to speak with a manager or supervisor and she would not let me.  Still a box and shipping label never came. She went into the Verizon store and they gave her a box but said they couldnt give her the shipping label  because customer service never made her one. They gave her the box and was really nice at the store and said call them and have them email it to you. She called and they would not email it because now they said she had the phone since November she had to buy it out. She told them thats why I called and ask and you pulled up my info and told me to mail it and i would owe nothing. I am a single 19 year mom and dont have 500.00 if you would of told me that I would have kept the phone. Lady did not care. I think that this is all so crazy that you get told one thing and then something else happens. Aint all the calls recorded on there end. Cant they could listen to what was told to her from jump and they should have to honor what was said. I sorry but verizon has gone down hill and needs to step up their game. Also how do you even get in contact with a manager/supervisor at verizon if you are told no on the phone.
