Hot Spot Notifications, but it isn't enabled!

I have gotten several notifications that one of the lines on my plan has used up the allotted hotspot allowance.

However, NONE of the phones on my line have been using this feature!
It is turned off on all phones.
What can I do to make sure I don't get billed for an overage on something we are not using?
