New line added without permission

A user on my account needed a new phone and went to the store to get it. I spoke to an employee and gave my permission to use an available upgrade on our account to allow this to happen. When reviewing the paperwork I was sent this employee also allowed this user to add an Apple Watch and attached a new phone line to the account without my permission. How do I get this Apple Watch and new line off of my account?
