I believe you have a limited amount of time to file for a mail-in rebate. The information about how to file, and what the time limits were should have been given to you when you purchased your phone. You should call CS and explain the situation and see if they can help you out.

Rebates do have to be in by a certain, as well have the UPC barcode from the box and the receipt. If you did all of that then you need to call the rebate center. Regular customer service can not process rebates as they are actually handled by a separate company.
Here's their number: 1-800-457-0864.

call customer care and ask them to call parago (the company that processes rebates). they should have some sort of documentation of your attempt to get a rebate on your phone. if the rebate has been denied and it is beyond the time frame in which you can resubmit then there is an escalation procedure by which they can credit your account. BE PREPARED FOR THIS TO BE A LONG CALL as the rep will need to thoroughly exhaust all avenues before they can issue a credit of that amount.
also, lay off the caps lock and learn to spell and punctuate. thank you.
