Schedule a callback is not working

The Schedule a Callback feature to reach customer service does not let you choose a date or time for the callback.  No callback can be scheduled.

If the schedule a callback feature is not working because of covid it would be helpful to leave a note on the page that the feature is down.  If this is a browser compatibility issue that would also be a helpful message to leave on the page.
