Just to start with, I'm somewhat familiar with this process if it's for a recent order or a replacement, but if this is for something else, like a rebate, the information below probably won't be helpful to you.
Was one supposed to be include with a package you received? If so, but you didn't receive one, then you should be able to print one from the Verizon Wireless website. From the menu bar at the top, navigate through the following:
My Verizon > My Devices > My Orders
Once there, if you select your order, then you should have an option to print a return label. If you're not able to print for some reason, then I'm afraid your only option will be to directly request Verizon ship you a replacement, though it will probably speed things along if you have your order number available when you call them.
