I have tried every outlet and forum to get this information with no success. Can someone please tell me what shipping address I should use to send my device back?
I wish I could tell you. If you can still login to My Verizon, you can print a return equipment label from there. But if you've disconnected all your Verizon services, there is no way to login. The CS reps haven't had much luck either with getting labels to disconnected customers and I'm starting to wonder if it's because they also cannot access a disconnected customer's account?
