HTC Desire 636 Puke Code Help

I need help finding my PUK code for my Phone and so far no help from the Knowledge base "Locate the 4G SIM PIN/PUK - My Verizon Website | Verizon Wireless" It does not explain for prepaid users and no help for my device. Can anyone help beside going to Customer service for this? There should be a place on this website to find my PUK code. Thanks in advance!

Here is what I see and what I am stuck at. I can't find the PUK Code link that everyone is telling me. Also I cannot click the picture to find the link. It is just a PNG.

You have to call the prepaid customer service from another phone to the PUK code for your phone. (888) 294-6804
