Do installed apps *and their data* transfer to new phone?

I have a Note3 and apps that I have used for hiking and monitoring exercise heart rate.  How do I transfer that data to my new Note7 ??

i used samsung smart switch to xfer, contacts, text messages, apps from an s5 to a note5 on wi-fi.

all the pics wouldn't xfer phone 2 phone but i used smart switch to xfer music & pics from pc using wi-fi on both.
