No, it does not. Taken from VZ Wireless:
Verizon Wireless Extended Warranty (EW) provides convenient coverage against mechanical or electric defects after the manufacturer's warranty expires. This option does not cover lost, stolen or damaged devices.

warrenty covers a manufactor defect. there can not be any physical or liquid damage to your phone or it voids manufactor warrenty. they basically send you out a certified like new replacment over night and you send them back the broken phone. do not lie to them and send back a broken phone because the charge for sending back a broken phone is probally more than just buying a new one

Warranty no, there is usually no cost. No physical damage or they won't accept it.

Verizon has 2 plans, Extended Warranty which simply extends the Manufacturers Warranty, and Insurance (Asurian) Which is an Insurance Policy and will cover most things but there are exclusions. Extended Warranty is no Deductible, Insurance has a Deductible any where from $75.00 - $200 depending on the phone they are covering

There can be ZERO damage to the device. Physical or water damage nulls the warranty, but this is great if you have a software glitch after the 1 year manufacturer warranty expires. It's only $3 a month and the replacement costs nothing
