how do i get ahold of the insurance people on my phone i went to verizon the other day they ghave me a card to call and i lost it

my phone is a droid razr HD screen is broken so i went to verizon to get a replacement they gave me a card to call because i have another insurance claim on it then i lost the card does anyone know what im talkin about or had a similar problem
