I am a deaf individual previous owner of tmobile, my parents decided to gift me with a samsung galaxy  4g just received today and already it *****. First I couldn't get into internet or applications to download for free relay or vphone for the deaf. Yes, i have unlimited service. Now I tried to create a screen lock, and what happens the whole phone locks asking for some sim puk code and no longer have any access to **** phone. Didn't even get to use the **** phone for 5mins and is already locked. The worse part being over 12 hours away from home have to waste income to ship **** phone 1 day express in hopes my parents return and order my tmobile cell instead ugh Smiley Sad

for a discussion.

  1. 1. Sign in to My Verizon.
    2. Locate the I Want To... section.
    3. Click More Actions.
    4. In the DEVICE column select Phone Details.
    5. Select Unlock SIM (PUK) Now. The Default PIN and Personal Unblocking Key display.

Thanks breezex, I did all that unfortunately I am not on the main account since phone and plan payment is a gift to me. So the PUK is blocked. I contacted verizon, but they still refused to give me PUK, that I must be added as a account holder. Told me to call my parents all I could say helloooo I am deaf teh whole point they sent me the phone I am living 12 hours away from them, and they in mide 60's and 70's not pc savvy, and no internet. so far is been 24hours and service as suckk since can't use the phone Smiley Sad

Verizon should be able to call your parents (or the account owner) and get permission to release the PUK code to you.  I'd try again and see if it's possible.  This is a user to user help forum and unfortunately in your case Verizon employees do not monitor these posts often enough to jump in and see if there is something that they can do.
