How do I stop my phone from unmuting itself?

I have the Samsung Galaxy Note 3 with 4.4.2.

I will mute my phone by using the notification bar and then, sometimes in a few minutes and sometimes an hour later, it will be unmuted. 

I'm still trying to see what actions are happening when this occurs but, so far, it will happen when I'm doing nothing.  This morning, a call came in that I ignored and after, the phone was unmuted.

Any ideas and/or suggestions would be greatly appreciated.

Help has arrived! When silencing the device, are you using the volume down key on the left hand side of the device to place the device on silent? If so, the device should remain silent until manually changed. When did this issue begin? Did this occur after any recent application downloads?

Hi Efrain,

I am using the Sound icon in the Notification Bar to place the phone on mute.  Shouldn't that work to keep the phone on mute until I toggle it?

I guess it's actually the Quick Settings panel I'm doing this in.
