Why does my account say my device is a non-vzw device?

I have been using my Note 5 since Aug. 2015, which I purchased from Verizon store.

Today, I found that my account shows that I'm using a non-vzw device.

I'm currently using my Note 5 that I purchased from Verizon store.

Is there anything that I can do to fix this issue?
