Thank you for helping me troubleshoot this issue. I verified all the GPS settings are correct, but it still does not work. There hasn't been any physical or liquid damage to the phone and the build number is LRX22C.I545VRUGOC1 on Android 5.0.1.
I have tried turning the GPS on and off, rebooting/power cycling phone and re-installing software but nothing to this point has helped.

Try taking the back cover off and make sure all the tiny screws are right,
especially the one on the top right (when looking from the back). That's
where the GPS antenna is located, pretty sure tightening it is what fixed
my phone. I've had to clean off my light sensor so the screen goes off
while talking (dust gets in through the speaker) and in my case it probably
wiggled out because the lock tite wore off.

You're very welcome, we are here to help! I appreciate the additional details of the troubleshooting steps you've taken. Have you tried downloading a third party application for GPS use to see if it works? Are all the other apps working okay?

Hi Karen -
I have tried several programs - Waze, google maps, GPS Test, GPS status etc and none of them can connect through my gps. All other non-GPS based apps are working correctly.

i received my new phone. gps is working just fine so far.
