How can you tell if your phone is being hacked or pirated?
Sometimes you can't, is there a specific reason to think this may have happened?

Well it's crashing quite a bit, running slow, I'm getting texts from friends that say they never sent them, Avast keeps crashing so I had to uninstall it, it says I'm using way more minutes than I really am, it's extremely slow especially when texting, visual voice mails are all staticy and when I make a phone call I hear a double click right before it starts ringing.
I doubt you've been hacked, but you can always do a factory reset just in case.

That is what I would do. Back up your stuff and do a factory reset. What phone do you have? You created this thread in the Galaxy S3, so if that is the phone you have, did you get the JB update? Did it start before or after that? It is sometimes good to do a factory reset after an OS update if you are experiencing problems that cannot be fixed.
