So since it's been more than a year it would need to have insurance coverage. Do you pay for this monthly on your bill?

I would take it to a Verizon store or a Best Buy with a Samsung Experience location inside it.

It would be pointless to take to a store as a store can do nothing in this situation. Even if it were within the 1 year warranty period would need to call in to get Certified Like New Replacement ordered.
If it is outside of the 1 year warranty there would need to be insurance on it (and would need to then contact Asurion by phone to file a claim or via phoneclaim.com/verizon).
