I pre-ordered at the Verizon store for two Galaxy s8+. Part of the deal was to trade in my two Galaxy 6. I have not received the mailer to send the phone in, how do I do this?

(http://vz.to/2aMleRk) Once you fill out the questions regarding the phones you are trading in, there is a portion where you enter in your address to have the boxes sent to you.
