Calls not registering on Samsung Galaxy Nexus

I have had the Samsung Galaxy Nexus since day one 12/15.  In the last month or so I have been having an issue where friends/family/work will try to call my cell phone it will ring a few times then go to voicemail, however the call never even registers on my handset at all.  I called VZW about this issue, they suspected that my device was possibly faulty so they sent me a replacement unit, everything seemed to be ok. 

Until today, I started having the issue again, I even did a factory reset on my handset and have been  able to replicate the issue, it doesn't happen 100% of the time but I'd say at least 30% of the time if not more.  This is very annoying.  Seems odd that my second handset would be doing this as well, so I don't think its necessarily faulty hardware at this point.

Has anyone else had a similar issue?
