Galaxy Nexus will not complete OTA Updates

I tried searching the forums for anyone else that seemed to have this problem and I couldn't find anything.

I have a Galaxy Nexus and am still running Android 4.0.4, because I cannot install any OTA updates.  I constantly have the system update notification (at least once a day) and I can download the update without any problem, but when I restart & install, the update gets about 20% done and an error occurs.  I have full wi-fi and full 4g signal, but this makes no difference.  Anyone else have this problem that found a fix?
