I've had the phone since 5/5/15, and it has worked fine until today. I went outside for a while to shovel snow, and when I came back in, the screen auto-rotate had stopped working. If there's a way to check the onboard sensors, I'd like to do that before bringing the phone in to the Verizon store, since it's still under warranty. I tried entering *#0*# in the dialer, but nothing happened, so I assume that Verizon has locked that feature out.

It may sound basic but have you checked to make sure the screen rotation lock isn't on? Pull down the notification panel at the top and tap on Auto rotate. Tapping it once will put lock it on portrait and tapping it again will unlock it and place it in auto rotate mode. Outside of that I'm not sure what else it could be.
