What if device has been lost? The Asurion Affidavit requires you to supply ESN/MEID/IMEI. How are you supposed to supply that when device has been lost??

If you're logged into the desktop version of My Verizon as Account Owner/Manager, click Devices at the top of the page. Choose the appropriate line, then scroll down to IMEI.
