What is a QAM tuner in a tv. Do most come with it or is it more expensive do small sets have that tuner?

QAM is the method of digital delivery used by cable systems (including FiOS).
Most new TVs come with QAM tuners. Almost every TV with a ATSC tuner also includes a QAM tuner, but you should check the specifications to be sure. If you still aren't sure, you can ask here about the TV model.
On a TV with a QAM tuner, you will be able to tune local SD, local HD, and music channels on FiOS without a set-top box. For cable channels like TNT, USA, and ESPN, you'll still need a Verizon STB/DVR or a CableCard DVR like the TivoHD.
