Please send resolution for this issue noted in title. I have gone through all steps to resolve for this model prior to this message and none resolved my issue. I cannot hear the caller without turning on the speaker phone. I have taken the case off, checked settings, volume, etc.
Kindly help me resolve or send information for insurance claim. Getting quite frustrated!

There will be no need for an insurance claim if there is no physical or liquid damage to the device.
Call VZW or go to a corporate store and they can send you a replacement. If you are outside of the 1 year warranty you MUST have Extended Warranty, Total Equipment Coverage or Total Mobile Protection on your account. If you do not have one of these options then you are not likely to receive a no charge replacement.
If you have any physical or liquid damage and you have Total Equipment Coverage or Total Mobile Protection on your account, you should call Asurion and make a claim and pay the deductible.
