How do I talk to a supervisor?

How do I get in contact with a supervisor?  I have waited months for my phone trade-in credits and have spent hours talking to multiple support agents and employees in the stores.  No one has been able to solve this very straightforward issue and no one will put me in touch with anyone who can help.  I have to start all over and explain everything again anytime I talk to someone.  The support system still seems broken to me.

I am hoping Verizon Support is monitoring this and can help me.  I need this issue escalated to a supervisor, and the regular support staff has been unable to do this.  I was supposed to receive a call from a supervisor back in May but they never called me.  I would appreciate some help resolving this!  Thanks!
