How to reach someone that can help?

So  I traded in my old iPhone for an upgraded iPhone under the 800$ promotion.  The credit was never applied correctly and now I am getting charged every month for the new phone. I have spent countless hours on the phone complaining. They fix it and the next month the charge for a new phone reappears.  Despite numerous phone calls and reassurances that this has been fixed, no such luck. Where do I go from here?

Same thing happened to me.  Hard to stand by a company with business practices like this.
