Hello, I switched to Verizon from T-Mobile on 9/11/22, total 4 lines. I was supposed to get a rebate form for $200 per line or any notification on MyVerizon account but never received it. I contacted customer service numerous times for the form and I was told I should get an email with the form within a few days and it's been over a month. Very frustrating that I would have to call again and again for the rebate that I'm supposed to get a month ago.
