Phantom app won't stop asking for app approval

My daughter turned 14 this summer.  We switched plans and removed the family app from both of the phones.  My daughter also got a new phone at the same time and she's now on the 5G start plan (I'm on the Unlimited plan, not sure why).  We removed the app and updated the plans in-store.  The rep said it should have worked.  Her phone still won't allow her to download new applications without my approval.  How do we get it to turn off?  Is it a setting somewhere?
