Would be great if someone answered this question. Instead I'll spend hours on hold trying to ask the question. If I pick the wrong phone for the wrong box, probably wont get any credit.

After spending time on support call, I was advised that it doesn't matter which phone goes in which box as long as they both are sent back.

To be safe I am going to print each Submission and put it in the box as well just to make sure they know which phone belongs to which submission.
I'm having a similar issue. We need to return two phones as trade-ins to receive a rebate. I was only provided with one box, one sleeve and one shipping label. When I checked our account and the rebate status online, both phones are tied to the same shipping label. I have little doubt this will cause an issue and I'll only be given credit for returning one phone. I ordered both new phones (with both rebates for trade-ins) as part of the same order, which I'd guess would trigger the problem. Customer service has been of no help thus far in answering this sort of nuanced question. I need assistance right away. Please message me and help. Thank you.
