I had a Jetpack on a shared plan that was not being used because I didn't have the device anymore. Last year I purchased a new one that should have replaced the original device but I only just found out that it got added as a prepaid so I was charged for a prepaid balance and am still being charged for a jetpack on my shared plan.
How can I request a credit for the months I've been charged but didn't use?
You can't. It's up to you to review your bills. When you pay you agree to the charges. Besides when you log in it clearly shows the number associated with such.
