I don't know about uninstalling an app from a web page but if all else fails you could try recovering with the Nokia lumia recovery tool.

What bothers me is what app locked your phone that you cannot get into it...

Airlid's post is the best way.  Granted you'll be doing a hard reset which will wipe the phone completely; data and all.
