By default if you never set a pin it's the last 4 of your phone number

Is it asking for a puk code yet? If not, try some standard default pins, like 1111, 0000, 1234 and such until it says there have been too many attempts. At this point it will likely ask you for a puk code, which you can either get from MyVerizon or by calling in to customer care. It might also want to lock up completely instead, in which case there is a way you can just do a factory reset. I would need to know which kind of phone you have to tell how to do that.
