Perhaps a hardware problem, but more likely software.  Do you have other flashlight apps you can try or some you can uninstall?

Folks on Reddit seem to think a soft reset - or in some cases MULTIPLE soft resets - fixes it. After the GDR2 update lots of people had camera problems involving both the flash and just saving the pics when taken. A few of them returned their phones for replacements while others cured it with resets.
