Often Verizon will provide you with an unlock code for free, provided you have been a customer in good standing for 3-6 months.
Verizon doesn't have all the codes for all the phones they sell. If they don't have it for your particular phone you will have to find a commercial unlocking service and pay for a code. There is also a chance the code you buy won't work. That happens a lot.

Thanks for the response. I was ordering a sim card for use in Israel, and one of the caveats was that the phone had to ne unlocked for it to operate properly. I stopped by my Verizon store on Sunday and was assured that my phone was sold to me in and unlocked condition and the replacement sim should work. I’ll know Saturday when I get to Tel Aviv.
