How can I remove the name and number from my old phone before I get rid of it? Audiovox CDM9000

MusicMaster we'll walk you through the steps! To wipe the info from the device please:
1.Switch on the handset.
2.Then press Left soft key 5 3 on dial pad and enter your password. (Password is the last 4 digits of your phone number)
3.Now press 4 key for "Reset phone".
4.Select "Yes" and confirm with OK.
