How to remove "my number" from old iPhone 4

Hi,

I have switched from iPhone 4 to iPhone 5. I would like to sell the old iPhone 4 but it still has "my number". Also, it seems the iPhone 4 is still active (I can call to my office phone from the old iPhone 4) even if only iPhone 5 is active per my verizon online account... Please let me know how to deactivate the old iPhone 4. Best Regards,
