I had the same problem, and I followed all of the above steps and still got absolutely nowhere. After checking all settings on my phone, i noticed that I had a "profile" installed (under Settings, General). After removing the profile, my phone immediately began receiving and sending picture messages. I'm sure every phone problem is different, but this was the defining moment in fixing MY phone.
