How do I permanently deactivate my phones on line?
If you mean to cancel your account, you can't do that online, you must either go into a store or I believe you can call CS.
If you mean to deactivate the phone when activating another on your line, only one phone is active at a time. Once you activate a new phone, the old one is automatically de-activated.
