Activating an iPhone I got from Amazon

Ok! So the other day my Droid razr died. This is the 4th droid I've had so I bought an unlocked  iPhone 5 from Amazon. I have two questions!

1) How do I go about activating this iPhone?

2) Do I need my SIMS card from my old phone? (I switched to a 3g device so my SIMS card was deactivated.) What do I do?
