Can you still use an iPhone 5, as a phone while using it as a hot spot?

No. Any iPhone before the iPhone 6 cannot do voice and data at the same time on the Verizon network. It does not support HDVoice (also known as Voice over LTE VoLTE or advanced calling). This is required to allow voice and data at the same time.
