My son was playing with my phone, and now it says that my SIM card is locked. On my profile it says that my PIN number and PUK code are unknown. What do I do???
Solved! Go to Correct Answer
Log into your on-line MyVerizon account. At the top of the page where it says MyVerizon, hover over that and click on View All under My Device. At the top, choose the appropriate device, if applicable. Under About My Device, click on My PIN and Unlocking Key (PUK), and you should get a pop-up with the info.

I think I have read you can get the PUK code through your MyVerizon account online. Try a search of other similar discussions on this community forum.
