Well you need to call Customer Service and have them Give you the PUK Code you can get it as well if you have Established a UserName and Password to My Verizon. Once in there Look for PUK Code..
Here is Customer Services No. 1-800-922-0204 Option 3 for Tech Services.. Good Luck
Log into your on-line MyVerizon account. At the top of the page where it says MyVerizon, hover over that and click on View All under My Device. At the top, choose the appropriate device, if applicable. Under About My Device, click on My PIN and Unlocking Key (PUK), and you should get a pop-up with the info.
