The only thing I can suggest is backing up the device, restoring to vanilla and seeing if the problem is still happening before you restore to the backup. Apple will require this before they do any type of warranty exchange. I'm assuming the device is out of warranty. You can verify here: Apple - Support - Check Your Service and Support Coverage
