I've had my new iPhone 6 for 2 weeks. Periodically it says "no sim card". This disables the phone and I have to shut it down to reboot it. Anyone know what's going on? Am I doing anything wrong? Do I just have a bad phone?

SIM cards are free if ordered through Customer Service or if picked up at a Corporate VzW store. Third-party retailers/resellers usually charge for them.
Sounds like a bad sim card which needs to be replaced. Of course, VZW charges $20 for that.
To the best of my knowledge VZW does not charge for a SIM card. I've never been charged...
I was reading another another user on this forum who was charged for one so I mentioned it. Its better to go in expecting to pay and not have to pay anything than to go in thinking it will be free and finding out there is a charge for it.

They may have went to an independent 3rd party retailer (authorized retailer) where they can charge fees different than an actual VZW corporate store would. But an actual VZW store does not charge for a SIM card. Nor would you be charged if you called CS and requested one.

If you've only had your device for 2 weeks I wouldn't pay a cent for a SIM card. If I couldn't get VZW to give me one for free I'd be taking that phone back to where I purchased it from.
As already mentioned, VZW will give the OP one for free. It's "authorized retailers" that often charge for one but if you don't have a corporate store in your area you can call VZW to have one sent to you for free
