Who I have the same questioned this topic

I was offered a $250 Costco card per line I switched from AT&T it’s been over 90 days neither Verizon reps at Costco manager have an idea of how this promo works. Has anyone received these cards if so what is the process? I keep getting the run around from both companies. Please help!
