Callers cannot hear me unless i am on bluetooth or speaker

I am having a problem with my iPhone4 where callers cannot hear me unless I am on speaker or Bluetooth.  I've gone through the troubleshooting and settings without improvement.

Have you updated the PRL lately by dialing *228 and option 2? Are there any Software Updates available on the phone?
