How do I change my phone's name that comes up on caller ID?

When I call someone my phone name, on caller ID, comes up as my previous employer's name.  I recently left the company and took the number with me.  I transferred the account from ATT to Verizon but the caller ID comes up as my previous employer's name.

How do I change this?
