How do I re-activate my iPhone 4?

I currently have iPhone 5 but I want to give that to a relative and go back to my iPhone 4. How can I go about this?

dhong123, We can answer that question! You can activate the device online via My Verizon: http://bit.ly/xB4iTc . Make sure the iPhone 5 is powered off and dial *228 and press send on the iPhone 4. Wait until activation is complete then you can activate the iPhone 5 on your friend's account.
