It sounds like you already activated another phone, and now it won't let you make a claim on the old phone? They normally need you to make the claim while the old phone is still active. You may need to temporarily activate the old phone long enough to make the claim, and then activate your new device again.

Call the insurance company and tell them exactly which phone you wanted to replace.
